The Uniform Order Form is presented here in editable Microsoft Excel format.  If you wish to complete and return this form via E-mail, please send it to hms@holmemead.beds.sch.uk. Full payment is required before your order can be processed so please send your cheque into school as soon as possible.

Alternatively you can return a printed copy of the completed Uniform Order Form to the school office
with full payment.
